Transition of care to adult nephrology

The transition from pediatric to adult nephrology represents a critical period for adolescents and young adults with chronic kidney disease, Congenital Anomalies of the Kidney and Urinary Tract (CAKUT), hereditary nephropathies, and post-transplant care. This phase is often associated with increased risks of treatment non-adherence, graft loss, disease progression, and loss to follow-up. Structured transition programs are essential to promote self-management skills, health literacy, psychosocial readiness, and continuity of care. Multidisciplinary coordination between pediatric and adult nephrology teams, nurses, social workers, and mental health professionals plays a pivotal role in ensuring safe and effective transfer of care.
