Obesity-related nephropathy (ORN) has emerged as a significant and growing contributor to chronic kidney disease worldwide, paralleling the global rise in obesity and metabolic syndrome. Characterized by glomerulomegaly, focal segmental glomerulosclerosis (FSGS), and progressive proteinuria, ORN reflects the hemodynamic and metabolic burden imposed by excess adiposity on renal structure and function. Hyperfiltration injury, activation of the renin–angiotensin–aldosterone system (RAAS), insulin resistance, lipotoxicity, and chronic low-grade inflammation collectively drive kidney damage. Increasing evidence highlights the complex interplay between adipokines, oxidative stress, and mitochondrial dysfunction in accelerating renal fibrosis and decline in glomerular filtration rate (GFR).
