Obesity is a pressing global health issue, particularly in Saudi Arabia (KSA), where missed diagnoses hinder timely intervention and increase healthcare costs. At Taif Region Military Hospital, baseline data revealed that only 65% of patients with obesity were identified, contributing to SAR 5.2 million in bariatric surgery costs. Fragmented care and low patient adherence further complicated the problem. This quality improvement project aimed to implement a structured obesity care pathway in two military hospitals to enhance health outcomes and reduce bariatric surgery incidence. A standardized, multidisciplinary case management-led pathway was introduced, aligning with value-based care principles. This quasi-experimental study, conducted from January 2023 to August 2025, targeted 3,492 patients (ages 17–65, BMI ≥30) eligible for lifestyle modification or surgical options, employing consecutive Plan-Do-Check-Act (PDCA) cycles. Key changes included standardizing obesity diagnoses, assigning case managers, coordinating care, promoting multidisciplinary meetings, and enhancing the Lifestyle Modification Clinic (LMC). Results showed a 30% increase in obesity identification (p<0.001) and a 46.4% weight loss among non-surgical LMC patients (p<0.001). Bariatric surgeries declined by 62.8%, saving SAR 2.9 million (p=0.01). Follow-up and LMC enrollment improved (p=0.002), with surgical patients reporting better quality of life and satisfaction three months post-discharge (p<0.001). Avoidable emergency visits decreased by 15% (p<0.001). This pathway effectively improved early diagnosis and adherence, providing a scalable model for obesity care.
