Title : Community Assessment Using a MAP-IT Model Approach

Abstract:

The purpose of this presentation is to provide an example of how the MAP-IT Model can be used to complete a comprehensive community assessment. Evidence-based practice resources will serve as a framework for the proposed assessment model. An adapted template will be provided to demonstrate the phases of the model (Mobilize, Assess, Plan, Implement and Track) along with a presented example of an identified community issue. As the MAP-IT Model utilizes a comprehensive systematic approach, it can be adapted to any leading health indicator/public health concern and therefore is a valued tool that should be part of the healthcare providers repertoire.

Biography:

Wane has a PhD in Nursing Science, MSN and is a board certified FNP, with undergraduate bachelor degrees in nursing and nutrition. After 32 years in academia, Dr. Wane currently works as a Nurse Consultant. She has published supplements in textbooks, as well as journal articles, and serves as an editorial board member and peer reviewer for several journal publications. She is also a member of Sigma Theta Tau Nursing Honor Society.
