Abstract:

Alcohol Use Disorder (AUD) poses a significant threat to relational functioning, strongly linked in the literature to decreased marital satisfaction, heightened conflict, and communication breakdown. To address this, couple-based interventions have been developed, yet their models vary globally. This integrative review (2016-2025) synthesizes 97 empirical studies (91 English, 6 Chinese) to compare intervention strategies. The international literature (n=91) confirms Behavioral Couples Therapy (BCT/ABCT) as the gold standard, with robust evidence for its efficacy in reducing drinking and improving relationship quality. Alongside BCT, models like Congruence Couple Therapy (CCT) also show promise. Current international trends focus on adapting BCT into group, telehealth, and pharmacologically-augmented (e.g., Oxytocin) formats. In stark contrast, the Taiwanese literature (n=6) operates within a legal framework primarily linking AUD to intimate partner violence. Local interventions do not reflect the BCT model, instead utilizing mandated Cognitive-Behavioral (CBT) groups and Solution-Focused Brief Therapy (SFBT). This review identifies a critical gap between established international evidence-based practice and localized, context-driven interventions. This highlights an urgent need for the cross-cultural adaptation of BCT models for Taiwanese couples and specialized training for practitioners navigating the legal and clinical complexities of AUD in Taiwan.

Yen Hsin Yi, Ph.D., is an Adjunct Assistant Professor at the Graduate Institute of Counseling Psychology and Rehabilitation Counseling at National Kaohsiung Normal University (NKNU), Taiwan. She is also a Licensed Counseling Psychologist specializing in marital counseling and alcohol addiction treatment. Dr. Yen teaches courses including "Marriage and Family Therapy" and is a certified counseling supervisor recognized by both the Taiwan Counseling Psychology Association and the Taiwan Guidance and Counseling Association.
