Abstract:

Background: Platelet-rich plasma (PRP), a platelet-concentrated blood derivative obtained through centrifugation, has been investigated for ovarian rejuvenation since 2017, though its clinical efficacy remains controversial.

Objective: This study assessed the effect of autologous PRP on anti-Müllerian hormone (AMH) levels in women with diminished ovarian reserve undergoing in vitro fertilization (IVF) at Hanoi Obstetrics and Gynecology Hospital from August 2024 to February 2025.
Methods: An interventional study was conducted on 28 women with AMH < 2 ng/mL undergoing IVF during the study period. Changes in AMH levels after intraovarian PRP administration were analyzed, and outcomes were compared according to age and antral follicle count (AFC).
Results: After PRP treatment, AMH levels increased in 75% of patients, remained unchanged in 3.57%, and decreased in 21.43%. A significantly higher rate of AMH improvement was observed in women under 40 years of age compared with those aged 40 years or older (p < 0.05). No statistically significant difference in PRP effectiveness was found between patients with AFC < 5 and those with AFC ≥ 5 (p > 0.05).
Conclusion: Intraovarian PRP injection appears to have a positive effect on AMH levels in women with diminished ovarian reserve undergoing IVF, particularly in younger patients.
