Abstract:

Vital pulp therapy (VPT) has long been utilized as a treatment for immature permanent teeth diagnosed with reversible pulpitis. One of its main advantages lies in preserving the vitality of the pulp while avoiding the potential complications associated with conventional root canal therapy. Over time, advancements in diagnostic tools and biomaterials have influenced clinical decision-making, leading to an expanded application of VPT, even in more complex cases.

According to the American Association of Endodontists (AAE) guidelines, strict adherence to specific protocols is essential for achieving successful outcomes with VPT, particularly in mature permanent teeth presenting with irreversible pulpitis. This has sparked considerable interest in the endodontic community, raising the frequently debated question: Can VPT serve as a viable alternative to traditional root canal therapy in these cases?

Numerous studies and systematic reviews have explored the effectiveness, feasibility, and clinical outcomes of VPT in mature teeth with irreversible pulpitis. Findings suggest that while VPT holds promise, its success heavily depends on proper case selection, compliance with established protocols, and diligent follow-up. Therefore, VPT should not be universally adopted as a replacement for root canal treatment in such cases without careful consideration.

Ultimately, VPT offers a minimally invasive approach that may provide favorable outcomes when performed under the right conditions. However, to ensure long-term success and prevent treatment failure or complications, clinicians must strictly follow clinical guidelines and monitor cases over time. Only through evidence-based practice can VPT be effectively integrated into the management of irreversible pulpitis in mature teeth
