Title : Use of TCA 35% in androgenetic alopecia

Abstract:

Introduction and objectives: Despite numerous models exploring Androgenetic alopecia (AGA), there are only 2 FDA approved treatments (Finasteride and Minoxidil). The response in AGA reaches a plateau after a few years of treatment. Chemical peels can potentially be utilized in that phase as they cause therapeutic wounding and provoke growth factors that may induce hair regrowth. In our case study, we have used Trichloroacetic acid 35% (TCA) peel as an adjuvant in resistant cases of AGA.

Materials and methods: This study was designed as 20 weeks prospective, therapeutic trial. Eight male patients were enrolled in this study. Patients having AGA for average 3 years who had tried different modalities (invasive and topical) were selected. Left half of scalp was selected for TCA peeling agent application. Patients resumed routine treatment of 5% Minoxidil after 5-6 days of TCA application. 5 such sessions were done every 3 weekly. Photographs and dermoscopy was recorded at the initial presentation, after 3rd session and after 5th session.

Results: We report clinically visible hair growth on the side of application compared to the opposite side. There was dermoscopic improvement noticed after 3 sessions as well as 5 sessions assessed with a dermoscopic criteria.

Conclusion: We conclude that TCA peel can be preferred as a booster therapy in Androgenetic alopecia patients as it can provide rejuvenation of the scalp, that has been subjected to multiple treatments previously. TCA can act as an adjuvant for penetration of topical medications. It is novel, painless therapy, leaves no skip areas, with no systemic absorption unlike other chemical peels.

Dr. Payal Choithani is a Junior Resident in the Department of Dermatology at Seth G.S. Medical College and KEM Hospital, Mumbai, India. She is engaged in clinical dermatology training and patient care in a major tertiary-care teaching hospital, with exposure to a broad range of dermatological conditions and diagnostic and therapeutic approaches.

Her academic interests include clinical dermatology, emerging developments in skin disorders, and advancing patient-centred approaches to dermatological care. Through her clinical training and academic activities, she is developing her expertise in dermatology and contributing to the learning and research environment at KEM Hospital.
