Title : Yasser’s V1-AVR rhythm syndrome and diagonal biting compressive radiological Yasser’s sign-with a differential ECG dextrocardia, old anterior infarction, and suspected renal infarction-cardiovascular and radiological discoveries-A case report

Abstract:

Introduction: Anterior myocardial infarction has higher mortality and morbidity than other types. Dextrocardia is a cardiac position that is a mirror image of normal anatomy. In patients with dextrocardia, the standard 12-lead ECG is characteristic. Renal infarction is frequently missed or delayed because the patients present with flank pain that is similar to other common conditions. Case presentation: A 58-year-old male, Egyptian worker, was admitted to the intensive care unit (ICU) with CHF, hypotension, hematuria, and uncontrolled diabetes. Conclusion: "Yasser’s V1-aVR rhythm syndrome" is a newly described syndrome with a constellation of characteristic STE with preceding pathological Q wave, ending with slight shallow inversion in V1, biphasic P wave with a slight terminal portion in V1 lead, dominant small R with transient conversion to QS in V1 lead after management, PR prolongation sparing aVR, pathological Q waves in anterior and high lateral leads except V5 and V6 with some variations, a fixed RAD, four new types of QRS fragmentation, a transient QTc prolongation, a few transient PVCs with STE elevation and without. "Diagonal biting compressive radiological Yasser’s sign" is a newly described sign characterized by an irregular diagonal biting-shape in the right middle para-mediastinal region of the heart. It is associated with an irregular diagonal extension dilation-shape area in the opposite left middle para-mediastinal region of the heart. The right PA and the lower part of the SVC are targets for the first right diagonal, but the PA trunk is a target for the second left diagonal. The diagonals are decreasing in size with subsequent management. Future research will be recommended.
